Quick Reference Sheet: Adding New Users to your Entity’s Choice Partners Member Account

1. Navigate or go directly to the url: https://www.choicepartners.org/member-login. 
2. Click on the Set Up a New Account Link

   

                                         

3. A box will pop up.  Begin typing the name of your entity.  As you type a list of entities will appear matching what you type.  Select your entity. Then complete the requested fields of information: Address, First Name, Last name, title, email, and phone.

   

                          

 

4. At the bottom of the form enter a username and password, then confirm the password.

   

                        

 

5. Allow a business day or so for Choice Partners staff to confirm/validate your information.  Call us at 713-696-2122 or Toll free at 877-696-2122 to check on your new user account request, if your request is needed urgently or for any assistance
